What the service includes
Home environment assessment
We visit the person's home and assess how their current functional limitations affect their ability to move through, use and manage the space safely and independently.
Barrier identification
We document specific barriers to access, safety and independence within the home — from entry and bathroom access through to kitchen and bedroom function.
Simple modifications
We recommend low-cost, non-structural changes where appropriate — grab rails, non-slip surfaces, threshold ramps, lever-style door handles and handrails to steps.
Complex modifications
Where more significant changes are needed, we assess and recommend structural modifications such as bathroom reconfigurations, ramp construction, doorway widening and hoist installation.
Clinical report writing
We write the detailed clinical reports required to access funding for modification works, formatted to the standards expected by the relevant funding body.
Coordination support
For complex modifications, we work alongside builders, project managers and the funding body throughout the process to ensure recommendations are delivered as intended.
