We provide practical, personalised therapy that supports people in everyday life, working across a range of funding streams including WorkSafe Victoria, the Transport Accident Commission and the National Disability Insurance Scheme.

How it works

The referral and service process varies depending on your funding stream. Select yours below.

A referral is made

A GP, support coordinator, case manager or the person themselves contacts us. We accept referrals online or by phone.

We confirm and make contact

We review the referral and contact the client within one business day to schedule their first appointment.

Assessment and reporting

We complete the required clinical work and documentation. Reports are delivered within 10 to 14 days.

Make a referral
No agent pre-approval needed for the first sessions. Once a GP refers the injured worker, initial treatment can commence. We handle all Allied Health Recovery Plan documentation and submission.

GP refers the injured worker

A WorkSafe approved GP refers the injured worker directly to REM Healthcare. No pre-approval from the WorkSafe agent is needed.

We contact within one business day

We reach out to schedule the first appointment. The referring practitioner is notified when this is confirmed.

Treatment and recovery planning begin

We complete an Allied Health Recovery Plan and submit it to the WorkSafe agent for ongoing funding approval.

Regular updates throughout

Progress updates are provided to the GP and case manager at key milestones throughout the recovery process.

Make a WorkSafe referral
No pre-approval needed in the first 90 days. TAC covers the cost of an initial OT assessment within 90 days of the accident. Clients can self-refer, or their doctor can refer them.

Getting home safely

Many clients come to us when they are ready to leave hospital but unsure if their home is safe. We assess, identify what needs to change and write the recommendations.

Rebuilding everyday independence

An accident can affect things you never thought about, getting dressed, preparing meals, managing the home. We work through these practically and at your pace.

Documenting what you are entitled to

We conduct a thorough assessment and write a detailed report accurately reflecting how the accident has affected your daily life.

Home assessment and modifications

We assess the home and write the report TAC needs to fund modifications, ramps, rails, bathroom changes and more.

Equipment prescription

Assessing what will genuinely help and prescribing the right aids and equipment for recovery and independence.

Detailed functional assessment

A thorough report documenting exactly how the accident has affected your daily life.

Capacity building sessions

Ongoing sessions to rebuild skills, cooking, community access, managing the home.

Self-managed and plan-managed participants welcome. We do not currently work with NDIA-managed participants, but all other management types are supported.

Your NDIS support coordinator or a family member can refer you. You can also contact us directly to discuss your needs.

We review your NDIS plan together

We confirm which supports are funded under your plan and which service agreement applies before any sessions begin.

It depends on how you are accessing the service. For a private booking, no referral is needed and you can contact us directly. For WorkSafe, TAC and NDIS, the referral process varies slightly. Contact us and we will walk you through what is needed for your specific situation.
A functional capacity assessment is a structured evaluation carried out by an occupational therapist that documents how an injury or health condition affects a person's ability to manage everyday life, from personal care and domestic tasks through to work and community participation. The OT observes the person completing tasks directly, uses validated outcome measures, and produces a comprehensive written report. It is one of the most commonly requested OT reports in Australia and is used across a wide range of funding and clinical contexts.
Yes. We support both children and adults across all of our services. This includes sensory profile assessments, functional assessments and capacity building programs for paediatric clients, as well as the full range of services for adults across all age groups.
Yes. We conduct home modification assessments for clients across a range of funding streams and for private clients. We visit the home, assess what modifications are needed and write the clinical report required to access funding for the works, including grab rails, ramps, bathroom modifications, doorway widening and more complex structural changes where needed. Some of our occupational therapists have completed additional specialist training to support more complex home modification assessments and prescriptions.
We respond to all referrals within 1 to 3 business days. In most cases we can schedule an initial appointment within two weeks of receiving a referral. If you have an urgent situation, for example a client being discharged from hospital, call us directly and we will do our best to accommodate the timeframe.
